Official Food Vendor Application Form
Please Note: To celebrate our 46th year we will be featuring Mobile Food Vendors, including Mobile Food Trucks. Other Food Vendor spaces will be limited. All applications, information and fees must be submitted by March 10th.
All food vendors must be approved. Please fill out this application, pay the fee and submit. You will be contacted shortly. At that time if you are approved you will email your insurance papers back to the email that tells of your approval. If you are not approved (usually due to having too many of the same type of food) you will be notified and your fee will be refunded in full.
Insurance: The Rotary Club of Thousand Oaks does NOT provide liability or property insurance coverage for vendors. Claims resulting from actions by a vendor or incidents related to a vendor’s booth are the liability of that vendor. Please contact your insurance agent and have them prepare a Proof of Insurance (POI) that you will email to us or if you need to purchase insurance you can contact: https://www.theeventhelper.com or other agent of your choice.
